UAB HEART AND VASCULAR CLINIC OF CENTRAL ALABAMA BILLING POLICY
We ask all patients to come to our office prepared to show their current insurance ID card, pay their co-pay, and to make any necessary payments at the time of service. We encourage all patients to understand their insurance coverage. We will file all primary and secondary insurance claims for our patients.
As required by your insurance company, you are responsible for the portion of your charges not covered by your insurance policy. Patient statements are sent monthly. Statements are not generated until your insurance company(ies) has processed the claim(s) submitted. We accept cash, personal check, money order, MasterCard, Visa and American Express as payment. For those patients who require extensive treatment or are uninsured, our Billing Specialist will be happy to speak with you to arrange a monthly payment plan.
Keep in mind that coverage varies significantly among carriers and policies. Therefore, it is the patient’s responsibility to thoroughly understand his/her policy and its coverage. It is important to bring your current insurance card to every office visit, and, if necessary, a primary care physician’s referral. If your plan requires a referral and you do not have one at the time of service you will be asked to sign a waiver acknowledging that you are being seen without a referral and will accept financial responsibility for the services provided. If you are unsure whether your plan requires a referral, please check with your insurance company or your primary care physician’s office prior to your visit. Always notify us immediately when you have a change in your insurance coverage.
We participate with most managed care plans. Usually, managed care plans require the patient to pay a “co-pay”. We ask that patients be prepared to pay this at the time of their visit. We also participate and accept Medicare’s assignment, and all claims will be filed by our office.
Please see the list below as a general guideline of plans with which UAB Heart and Vascular Clinic of Central Alabama participates. If you do not see your plan listed, you may be able to see a clinic physician using your out-of-network benefits, keeping in mind that your out-of-pocket expenses could be higher than if you use an in-network provider.
Please note: This list is subject to change. Please take the time to contact your insurance plan to verify the physician you are scheduled to see is a current plan participating provider for your policy. If you fail to confirm participation of a provider with your insurance company, it may result in your claim being processed with a higher patient cost sharing or denial of benefits.
